I have looked at the process again from the position of a normal ipernity member who actually just wants to have a good time here with friends. People don't want to read much and don't want to engage in much either. That's why I would shorten the text considerably. See the suggestion below.

In this version, you would only have to enter two data instead of the previous 19. In addition, you would not have to name a substitute admin, but could take care of the matter yourself as moderator within a reasonable period of time. I wouldn't set a specific deadline either. After all, the informed member can leave both the former and the new group at any time.

All of this would be a much more streamlined approach without anyone being left out.
